Once you click on the "E-Form" nearest your lab, you can:

1. Download the form to your computer by clicking "Save" and complete the order form from your own computer; or

2. Complete the "E-Form" online by clicking "Open." Once you complete the form, print the order and then fax it to your local Sleep Center. 

3. You can also save the completed E-Form to your computer for your own records, using the patient's name for the file name.

It is important that you do not email the form in order to remain HIPAA compliant.  Please FAX ORDERS only.
